CLINIC POLICY REGARDING DIRECT BILLING

ALL PATIENTS: Please note, we no longer provide direct billing services to insurance providers for extended health plans. All patients will receive an emailed receipt with your therapist’s licensing information to allow for submission for reimbursement to your insurance company when appropriate and in accordance with your policy and your insurance provider’s guidelines.

If you are experiencing symptoms of fever, sore throat, chills, coughing or diarrhea, please contact our office to reschedule your appointment.

I look forward to working with you to achieve your rehabilitation goals.

Sincerely, Rosie Chamberlin, MPT Licensed Physiotherapist
